The Crozer-Keystone Sleep Centers offer two different referral options:
1. Referring doctors may choose to send their patient for a consultation with a sleep physician (in which case one of our sleep physicians will order your patient's sleep study if necessary), OR
2. Referring doctors may choose to order a sleep study only (no consultation).
Easy Patient Referrals
For your convenience, physicians, physician assistants and nurse practitioners can download the Sleep Study Prescription Form for patient referrals. Simply fill out the form and Fax it to 610-340-2371.
Referring physicians, physician assistants and nurse practitioners may choose between the following two referral options:
1. Consultation with Sleep Physician
Before the Sleep Study: Patient or doctor’s office may schedule a consultation with a Sleep Physician by simply filling out our Sleep Study Prescription Form. During the pre-study consultation or initial evaluation, a comprehensive sleep history, medical history and physical examination is performed. For certain problems, during the initial evaluation, the patient also will be seen by a member of the Sleep Center’s psychological staff.
During the Sleep Study: Patient has Sleep Study at one of our three Sleep Center locations. Sleep technologists are present throughout the night to monitor a variety of physiological parameters. The patient is observed using ultra low-light video cameras and polysomnographic recording. Sleep staging, breathing, cardiac activity and neuromuscular variables are recorded.
After the Sleep Study: Patient sees Sleep Physician for a follow-up visit to review study results and to determine a treatment plan based on those individual results.
*Patient’s sleep care is managed by Sleep Physician.

2. Sleep Study Only (No Consultation)
Before the Sleep Study: Physicians, Physician Assistants and Nurse Practitioners may order a sleep study for their patient by simply filling out our Sleep Study Prescription Form. Along with that form, patient demographics & insurance card copies must all be provided to the Sleep Centers by the ordering doctor's office in to register the patient for the study. A pre-cert and / or referral is often times necessary, depending on the patients' insurance.
Crozer-Keystone doctors' offices who have access to Electronic Medical Records (EMR) may also order the studies electronically within the EMR system, under Orders. The Letters section must also be accessed in order to provide the patient's medical history. The Order and Letter fulfill the same requirements as the above mentioned documents.
During the Sleep Study: Patient has Sleep Study at one of our three Sleep Center locations. Sleep technologists are present throughout the night to monitor a variety of physiological parameters. After the Sleep Study: Results are sent back to the referring physician for a treatment plan to be discussed with patient, unless otherwise noted. *
*Patient’s sleep care is managed by the referring physician, unless otherwise requested by referring physician to have sleep physician manage patient's sleep care.
